Overview
Use this service to request a user code to set up your own account as a Survey Approver and/or additional codes for those who will need access as Survey Creators (who will submit to you for approval).
NOTE:
A Survey Creator can create new surveys but cannot distribute without submitting first to an Approver for review (students).
A Survey Approver can both create new surveys as well as review for approval or revision those of those submitted by Survey Creators (faculty/staff).
Eligibility
Nazareth Faculty,Staff and Students.
