  • Medical: You can schedule most medical appointments, request lab results, or ask questions online: Naz Student Health Portal.
  • Physicals: For an appointment for a physical exam, call 585-389-2500.
  • Counseling: For all counseling appointments or questions, call our office, 585-389-2500.

Summer Hours

During the summer session, we have limited appointments for enrolled students only. Call 585-389-2500 to schedule your visit. You may email us at with questions. We will respond as soon as we are able.

Student Health Portal

ALL NEW STUDENTS: 48 hours after your deposit has been paid, go online to Nazareth's Student Health Portal to input your required health history, immunization dates, and health insurance information. If your health information is not entered, you will be unable to start classes.

If you have trouble, or have entered your information and received a notice that your documents are incomplete, please contact our office.

Medical Excuse Letters

Nazareth's Health and Counseling Services will not issue notes, medical excuses, or visit verification unless an illness/condition is expected to last longer than 3 days.

Responsibility for class attendance and completion of assignments rests primarily with students, who assume responsibility by communicating directly with faculty via email or phone.

Chosen Name and/or Gender Identity Change

To submit a secure electronic request for a "chosen first name" that will show on class rosters, Moodle, ID card, and more, see "Chosen Name and/or Gender Identity Change" on the Registrar's Forms and Petitions webpage. After you submit the form, Health and Counseling will be notified of the name change. (Your legal name will be displayed on your electronic health record, but your chosen name will also become part of your record and used by our staff.)
