Qualifying Events

Changes are only permitted during annual open enrollment or within 30 days of a qualified status change. 

Qualified event/family status changes include:

  • Loss of coverage through another plan (employer or direct bill)
  • Marriage, divorce, legal separation or termination of domestic partnership
  • Birth or adoption of a child
  • Death of a spouse or dependent child
  • A change in employment status by you, your spouse, or eligible domestic partner
  • A child (including that of a qualified domestic partner) no longer qualified as an eligible dependent
  • Dependent's loss of Medicaid/CHIP coverage because of ineligibility
  • An unpaid leave of absence for you or your spouse
  • A significant change in health coverage for you or your spouse or eligible domestic partner, attributable to your spouse's or domestic partner's employment

Benefit/Plan changes are allowed when adding a new dependent (birth, marriage), but plan/benefit changes are not allowed when removing or cancelling a dependent (divorce, age off).

Please note: Documentation to confirm the status change will be required and any election change you request due to a "change in family status" must be consistent with the family status change that occurred. Please send your documentation to BenefitsHR@naz.edu
